DDS To Do List - 4th Year

  • The "TO DO" section of the Orientation website will provide you with information regarding requirements that all 2nd 4th Year DDS students must complete by the beginning of the main Fall term in August.
  • The chart below will list out the items you must complete. As information becomes available on how to complete each requirement, the item name will contain a link to detailed information. 
  • Please complete each requirement by the due date specified.

How to confirm if your immunizations were received?

  • Once your immunization records have been received, processed, reviewed and deemed compliant with requirements, your health holds will be removed; this will be reflected in your HUB Student Center in real time. This usually occurs 5-10 business days after we receive your compliant records (although it can take a few extra business days during the 3 months prior to the start of a semester).
  • Students will not be individually contacted to confirm receipt of records. Please check the HUB system (NOT your Patient Portal) for the current status of your holds.
  • If your paperwork is not in compliance, you will receive an electronic message  from Student Health Services. This message is sent to the student’s @buffalo.edu email address.

Academic Integrity Course

  • Maintaining a culture of academic honesty is essential for a successful learning environment. 
  • To ensure everyone understands our school's expectations for ethical conduct in the classroom and clinic, we require all students to complete a short online Academic Integrity course. The course will take 30 to 45 minutes and must be completed in one sitting.  This is an annual requirement.

Student Health Insurance

  • As part of your SDM Mandatory Training on UB Brightspace please complete, sign, date and upload the attestation form regarding your Health Insurance status. This is module 4. https://ubuffalo.brightspace.com/d2l/login
  • If you are not covered under the health insurance policy of your spouse, parent, domestic partner, etc. you must obtain health insurance coverage.
  • To ensure that you have the necessary resources and information to make knowledgeable decisions about health insurance coverage the university has provided several resources.  Please visit the Find an Insurance Plan page on the university website You can click the link to learn more about the NYS Health Insurance Marketplace or call 1-855-355-5777 for personal assistance.
  • In addition, the staff of the UB Student Health Insurance Office are trained to assist with the NYS Health Insurance Marketplace. Reach out with questions via email or call 716-645-3044. An in-person appointment can be scheduled with trained staff as necessary.
