Health Background Form

Due April 1, 2026

NOTE: You cannot take part in the IDP Summer Program until this requirement is fulfilled.

The university's Health Background Form must be completed by April 1, 2026.  

New York State law requires that all students provide proof of immunity against measles, mumps, and rubella. It also requires that all students receive information about meningococcal disease and make an informed decision about whether to receive immunization against the disease.

Please note: Dental students must also fulfill the requirements listed in the Health Professions tab under Meet the Requirements.

Submitting Your Health Background Form

All health and immunization forms should be sent directly to Student Health Services using the instructions provided on the Health Background Form website. Please contact Student Health Services with questions about the form. If time is a concern, you can call Student Health Services at 716-829-3316.

Please do not submit your health form to the dental school.
