The University at Buffalo School of Dental Medicine Periodontics Program accepts application materials through the American Dental Education Association, Postgraduate Application Support Service (PASS). This service enables applicants to send an originals of most application materials to PASS for distribution to all the programs that the applicant selects.

All application materials must be submitted by the application deadline.

  1. PASS Application. Apply through PASS.
  2. CV/Resume.
  3. Evaluation Forms. Three Letters of Evaluation (Professional Evaluation Forms (PEF) may be from academic faculty. Graduates of U.S. or Canadian dental schools must submit an Institutional Evaluation Form (IEF) or dean's letter.
  4. Personal Statement.
  5. Unofficial Transcripts from All Institutions. International candidates, please note: In addition to transcript copies, an ECE or WES Course-by-Course report is required. A copy of your certificate/diploma with proof of degree is suggested. 
  6. $50 Supplemental Application Fee. You are required to pay the nonrefundable supplemental application fee. Once we receive your application from PASS, you will receive a payment link.
  7. English Language Proficiency (for international applicants). In addition to the requirements listed above, international applicants must demonstrate English language proficiency via guidelines set by the university, as outlined on the Graduate School's website. A score report copy is sufficient. The UB School of Dental Medicine accepts the TOEFL with a score of 79 or higher, IETLS with a score of 6.5 or higher, or the Duolingo with a score of 120 or higher.
