About the Health Background Form
The university's Health Background Form must be completed by June 1, 2026.
New York State law requires all students to provide proof of immunity against measles, mumps, and rubella. It also requires that all students receive information about meningococcal disease and make an informed decision about whether to receive immunization against the disease.
Please note that dental students must also fulfill the requirements listed for health majors.
All health and immunization forms should be sent directly to Student Health Services, which manages all health documentation. Please do not send any health documentation to the School of Dental Medicine.
