Veterans CARES Program

The UB School of Dental Medicine's Veterans CARES (Counseling, Advocacy, Referral, Education and Service) Program assists U.S. military veterans with paying for select eligible dental services. The program is not income-dependent; all veterans are eligible. 

Veterans must be active, comprehensive patients at UB Dental and provide proof of veteran status through the DD214 form to qualify. Instructions for obtaining the DD214 form are available on the Veterans Administration website.

How to Apply

  • Current veteran patients may obtain a copy of the Veterans CARES Patient Agreement form from their provider or at our Welcome Center.
  • New patients may apply after completing their screening appointment, which is not covered by the program. 

Once completed, please return the patient agreement along with a copy of your DD214 to the Welcome Center. The effective date of coverage will be the day both documents are received and added to your chart.
