Health Background Form - DUE IMMEDIATELY

  • New York State law requires that all students provide proof of immunity against measles, mumps, and rubella. 
  • It also requires that all students receive information about meningococcal disease and make an informed decision about whether to receive immunization against the disease. 

 

You cannot be registered for classes until you have fulfilled this requirement.

The easiest and fastest way to become compliant with requirements is to make an AIR appointment at UB Student Health Services a minimum of 2 weeks in advance of their program’s deadline; 4 weeks in advance is better. Please call 716-829-3316 for an appointment.

Students who choose to go to an outside provider for services must submit the AIR form to SHS in person, via fax or through USPS mail. Student Health Services cannot accept records via email.

Students should submit records to Student Health Services directly. Please do not submit the health background form to the School of Dental Medicine. 

Submission of records does not automatically mean compliance.  Records need to be reviewed by Student Health Services staff to ensure all requirements have been met before a student can be marked compliant in UB Learns.
